To submit your claim please complete all the below fields.

• Please ensure all details are completed correctly as your claims are approved based on this information.
• Please ensure a copy of the sales invoice is uploaded along with your claim.
• A separate claim must be submitted for each invoice.
• One claim can be submitted when multiple units of the same product appear on the same invoice.
